At Human Care GP, we believe the best healthcare comes from a strong, ongoing relationship between you and your doctor. MyMedicare is a new, voluntary program that helps formalise this connection, ensuring you receive the most supportive and consistent care possible.
Why should you register?
By registering with Human Care GP as your home practice, you unlock several benefits designed to make your healthcare more accessible:
Longer Telehealth Consultations: Access Medicare rebates for longer phone consultations (20 minutes or more), making it easier to discuss complex issues from home.
Greater Continuity of Care: It helps us, your regular healthcare team, stay better connected with other parts of the health system (like hospitals) to ensure everyone is on the same page regarding your health.
Enhanced Support for Chronic Conditions: If you have a Chronic Disease Management Plan, being registered ensures you can continue to access tailored care and specific Medicare incentives through our clinic.
Who is eligible?
You can register with Human Care GP if you:
Have a Medicare Card or DVA Veteran Card.
Have had at least two face-to-face visits with us in the last 24 months.
Note: Some exemptions apply for people experiencing hardship or those living in remote areas. Parents and children can also be registered together if one person meets the eligibility.
How to register
Registration is free and only takes a few minutes. You can choose the method that suits you best:
Via the MyGov App: Log in to your Medicare Online Account or the Express Plus Medicare App. Look for the "MyMedicare" section and search for "Human Care GP."
In-Practice: Next time you visit us, just ask our reception team! We can start the registration for you, which you can then simply confirm on your phone.
Paper Form: If you prefer, we have paper registration forms available at the front desk.
Does this mean I can’t see other doctors?
No. You are still free to visit other clinics or see other specialists. Registering simply identifies Human Care GP as your primary health home, which allows the government to provide extra support for the care we provide you.
